		Rental Application Please fill out the rental application below (include every person over the age of 18), which must be accompanied with a copy of your driver’s license. There is a non refundable application fee of $40, payable by money order or cashier’s check.
		Authorization To Release Information This form must be signed along with your rental application.
		Notice Of Intent To Vacate Please submit this form to the property manager in person or by fax, if by fax you may want to call to confirm it was received.
		Rental Repair Request You may submit this request in person, mail, fax or email. All repair requests will be processed in order of receipt or level of emergency. Please allow 24 hours, not including weekends or holidays, for response. If you have not been contacted in that time, then please contact us on our direct line (225) 664-7000 to confirm that we have received your repair request.
